Account Name
Account Address
Prefix Code
Services (including description/instructions)
Custom fields (and order)
Category/Special fields (if needed)
Locations (if needed)
Schedulers name/email info
Provide information for vendors/suppliers

Link will be used to invite requesters to set up their own account.
